Flu season is here again. We are currently out of Seasonal Flu Vaccine. Please check our Facebook page for the most up-to-date information regarding availability. When we do have supplies, please be sure to download the form from the links below, print and fill out prior to your visit to save time.
Availability for the H1N1 “swine flu” vaccine is intermittent, but again, we refer you to our Facebook page for our most current supply levels.
Our seasonal flu vaccines are only for patients 18 and older.
